Supported Living Accommodation

Our accommodations are high quality and fitted with all the essentials for independent living with a warm welcoming family feel.

Independent tenancy agreement
Support is provided in respect of applications for housing benefits
Each room has its own en-suite bathroom maintaining each service user’s privacy

Personalized Support Plans

Each service user will have comprehensive support plan with several long-term objectives. These support plans will be reviewed on monthly basis and regular reviews will be done by individual’s wider support team. Support plans will include:

Self-care support
Budgeting support
Access to community reach, volunteering services, employment or education etc
Engaging in independent and meaningful activities

24/7 Care and Support

We will provide high level of support and clinical input to everyone as per their individual needs. Our qualified staff members will design and implement a bespoke support programme for each service user. Further support will be provided by:

A Registered Mental Health Nurse
A Registered Learning Disability Nurse
Experience Service Manager
24/7 support team
Out of hours on call services

Health and Wellbeing Support

Physical health and nutritional concerns are also monitored and included in the support plans

Promote exercises and self-care practices
Register with local GP if from another area
Our team will support with diet and nutrition and will always be at hand for any support in cooking and educational diet and nutrition.

Outcome Tracking

All individuals are assessed using reliable measures of mental state, cognitive abilities and daily living activities.

Mobile tablet system allows real time recording of activities and feedback
Daily records of activities and regular assessments
All data are restored safely
